States Senator is assisting my constituents who have questions,
comments, or problems related to the federal government.
Resolving the difficult situations that we sometimes face
when dealing with our government can be overwhelming.

the federal agency and need help working with that agency,

Information and Privacy Act Consent Form. If
you have questions or would like to express an opinion concerning
our government, you may contact

note that all federal agencies require that I receive
written permission before an inquiry can be made. Therefore,
it is necessary that I receive written permission from
the person of legal age that is requesting assistance.
This can be accomplished by sending a signed Case
Information and Privacy Act Consent Form
to one of my state offices. To assist minors, I must
have the permission of the parent or guardian in writing.
